| Family Support Worker Connect Society requires an energetic, motivated and career minded Family Support Worker. The FSW would be responsible for planning, developing, implementing, coordinating, and evaluating the delivery of Family Support Services to children and their families. The incumbent will provide leadership and clinical case management to the Family Support team. The incumbent will also be responsible for the provision of specialized home and community based support and prevention services to children and their families, as well as the successful delivery of programs that promote healthy lifestyles and relationships for youth. The incumbent also provides consultant services to regional staff and assists in the coordination and provision of community development and educational services. The incumbent must be fluent in American Sign Language (ASL). Job duties include providing a safe nurturing environment, assisting with personal care, documenting daily activities, attending regular staff and other meetings, being involved in Individual Program Planning (IPP's) for the child, attending ongoing workshops and continuing education as required. The FSW may work from a central office, travel to meet clients, or temporarily live with clients and/or their families.
